Implants

Discuss the advantages of the use of implants to create better retention and stability in mandibular complete dentures. What are the vital factors that have to be taken into account when choosing a specific implant.

  1. Basically implants serve as a foundation for conventional prostheses which must be fabricated after the implant has been inserted in the mouth. There are two different types of implants forms and depending on the manner in which the dentures are attached to the implant. Endosteal , which are implants that can be inserted into the alveolar bone,they are often used where there is sufficient height and width of the ridge. Then there is subperiosteal implants which are placed on top of the resorbed alveolar ridge.
    The advantages of using implants to create better retention and stability :- Where there is lack of tissue contact. there is no need for mucosal support due to the different forms of implants available. The implant abutment unit supports the denture entirely, therefore resulting in the denture not moving completly because there denture is not being supported by movable oral tissue therefore it is stabile in the mouth.
    -Less resorption will take place and the resultant ridge is preserved and the implant is adapted to supporting a complete denture.
    -Due to the denture being stable in the mouth there will be an increase in masticatory efficiency

  4. The advantages of using implants in place of a mandibular denture are vast. The greatest advantage is the stability implants provide in comparison to mandibular dentures. Implants can only be opted for when the patient has healthy oral conditions. Implants provide greater stability as it is embedded into the alveolar bone, the alveolar bone and surrounded tissue grows and encloses the implant or implants. The alveolar bone and surrounding tissue gets stimulated by mastication, food and other oral inhabitants. With good oral hygiene implants have greater longevity than a denture.

    Implants have greater biocompatibility as compared to dentures as research has proven that over a period of time dentures leach, implants provide greater aesthetics, phonetics,improved comfort, improved self-esteem, durability, the convenience of it being permanent mastication as well as overall hygiene. Implants have a greater success rate compared to acrylic resin dentures.

    There are two types of implants, titanium implants and zirconium. implants.Zirconium implants are considered the better implant as it has greater physical and chemical properties compared to titanium implants.

  5. Implants serve as foundations for conventional prosthesis which must be fabricated after implant insertion. Dental implants also function as natural tooth as it anchoring the replacement into the jaw bone. They also give better retention and stability to a denture during function.

  6. The vital factors that have to be taken into account when a choice for the specific type of implant is to be made are the following-
    - the volume of available bone
    - angulation
    - skeletal relationship of the jaws
    - bone width
    - anterior defects
    - arch form
    - cost (this to a lesser degree)

  7. the volume of the bone and the bone width will influence the size of the implant. It is feasible to have larger posterior implants to cope better with occlusal forces. The ideal size posterior implant would depend on the patient teeth size, however for the maxilla a 4,5-5mm implant is usually sufficient and for the mandible 5mm and larger. A larger implant will distribute forces better and will be more stable during excursive movement, specifically bruxism.
